Individual Therapy
Sessions can be scheduled weekly, biweekly, monthly, or as needed depending on your need, and are 55 minutes in length.
You must be physically in Pennsylvania at the time of the therapy session.
Kelsey Chizmar Therapy, LLC accepts the following health insurances:
- Aetna
- Anthem
- Blue Cross / Blue Shield plans: Many out of state BCBS plans are accepted too! Such as Massachusetts, Georgia, Michigan, etc.
- Capital Blue Cross Pennsylvania
- Cigna
- Geisinger
- Highmark Blue Cross Blue Shield of Pennsylvania
- Humana
- Independence Blue Cross Pennsylvania
- Oscar
- Optum
- Oxford
- Quest
- United Healthcare
- UPMC
I’m also able to accept a few other third-party insurance carriers, some out of state plans, along with plans whose mental health benefits are administered through some of the larger insurance networks mentioned above, and some select Medicare plans. There are also a few plans within those insurance carriers listed that I’m unable to take. Insurance can be unpredictable in which plans I accept and don’t. You can add your insurance information here and the benefit verification tool will be able to check your information.
You also have the ability to self-schedule yourself through that same link once your insurance is verified. I can’t access your insurance information until you register as a client there.
**I bill UPMC independently, so if you use the insurance benefit verification tool, it will say “out of network”.
If your plan is out of network, we are able to potentially use out of network benefits, self-pay or sliding scale options.
However, if you’d rather have an in-network Therapist, I can help you find the best fit!
I am unable to accept Medicaid plans at this time, but I am hopeful that in the future I will be able to!
Self-pay is at a rate of $130/session. Sliding scale options are also available.
